Please complete this pre-screen form if you are interested in more details about the PARIS test, SEngine collaboration, research questions, or clinical indications. Please note: If the patient is currently undergoing any cancer treatment and is responding well, SEngine is unable to accept samples. If recurrence, SEngine can perform the drug test after 2-3 week without treatment. This form is intended for patient inquiries about the PARIS® Test. If you are interested in collaborating with SEngine in a clinical or research capacity, please email connect@senginemedicine.com. PARIS® Test Eligibility Guidelines: • Confirmed cancer diagnosis with solid tumor refractory to treatment, progression, or stable disease • Confirmed locally advanced/rare cancer with limited therapeutic options • Confirmed three-week washout period of any interventional therapy (exceptions made on case-by-case basis) • Fresh, live biopsy tissue or liquid available for acquisition (must confirm with SEngine Team the specimen type, location, and date for biopsy sample kit shipment) The PARIS® Test Cannot be Performed If: • Patient has brain-only disease (Unless documented prion-free disease) • Fine Needle Aspirate biopsies are not enough material • Insufficient fresh tumor available for testing

I understand that the Treating Physician must complete the PARIS® Test Order Form prior to testing. I understand that I must provide SEngine patient medical, pathological, and genomic records prior to testing. I understand that my patient must sign SEngine’s Informed Consent Form, Patient Authorization Form, and Patient Acknowledgement Form prior to testing. I understand that SEngine requires 4 cores of fresh, unprocessed tumor tissue or 250 mL of ascites/pleural effusions. I understand that the sample can be obtained by core biopsy, punch biopsy, endoscopy, or surgical excision and must be shipped to SEngine in Seattle, WA within 48 hours of the procedure.
